Long-Term Care Ombudsman (your state's office)

Every state has an Office of the State Long-Term Care Ombudsman. It works to resolve problems about the health, safety, welfare and rights of people who live in nursing homes, board and care homes, assisted living and similar homes. It investigates and resolves complaints made by or on behalf of residents, and gives residents information about long-term services and supports.

Good to know

The ACL page links to a state-by-state finder run by Consumer Voice: pick your state to find the office and its number. A nursing home's written discharge notice has to give the name, address and phone number of the state ombudsman.

We list this organization because people told us it helped. We’re not affiliated with it, and we don’t speak for it. Last checked October 10, 2026: we opened its own site and confirmed this link and number. We don’t call the lines, and hours can change — their site has the latest.
